L’ajout du Sleep Analyzer à notre suivi personnalisé permet au patient une vue 360 de la qualité de son sommeil ainsi qu’une meilleure compréhension de l’impact positif de son traitement. Nous avons noté un meilleur engagement de ce dernier dans son traitement avec notamment une bonne observance dès les premiers jours, qui se prolonge dans le temps.

CONTEXTE

Asten Santé doit son origine à la constitution d’associations fondées dans les années 70 par des médecins pour permettre aux patients sous assistance ventilatoire d’être soignés chez eux et non plus à l’hôpital. Aujourd’hui, membre du groupe La Poste, Asten Santé intervient dans différents domaines comme l’insuffisance respiratoire, l’apnée du sommeil, le diabète, le traitement par perfusion ou encore la nutrition artificielle.

LES DÉFIS

Dans le cadre de l’apnée du sommeil, les équipes d’Asten Santé étaient à la recherche d’innovations afin d’apporter aux patients un service supplémentaire à la prestation de santé à domicile (PSAD) habituelle, tout en proposant aux professionnels de santé, une offre différenciante dans le suivi de leurs patients traités pour l’apnée du sommeil.

 

L’apnée du sommeil est un trouble qui se caractérise par une respiration superficielle ou des interruptions respiratoires plus ou moins longues pendant le sommeil. Aussi connu sous le nom du syndrome d’apnées-hypopnées obstructives du sommeil (SAOS), non soignée cette maladie peut engendrer à long terme des risques accrus d’hypertension, de maladies cardiaques, d’accidents vasculaires cérébraux ou encore de diabète.

A court terme, le manque de sommeil causé par le SAOS engendre aussi une somnolence diurne responsable de nombreux accidents de la route, une réduction de la qualité de vie et des facultés cognitives (mémorisation, productivité au travail…).

Le traitement le plus commun est l’appareil à pression positive continue (PPC), une machine fournissant un flux d’air sous pression constante à travers un masque pendant le sommeil ce qui permet de garder les voies respiratoires ouvertes (1). 

L’un des enjeux lors de la mise en place du traitement par PPC est, pour les patients, de l’accepter afin d’obtenir une adhésion maximale au traitement. En effet, selon l’étude ALASKA menée par ResMed sur une cohorte de patients français, l’observance des nouveaux patients est trop faible puisque 48% des patients arrêtent leur traitement avant 3 ans (2)

 

Asten Santé, en tant que PSAD et sous prescription médicale, met en place des visites régulières d’intervenants experts au domicile des patients, afin d’expliquer le traitement, installer l’appareil par PPC et faire un point sur les difficultés rencontrées. Dans ce cadre, les équipes ont réfléchi à un moyen innovant afin de mieux engager les patients dans leur traitement et leur faire prendre conscience de l’importance de ce dernier.

 

C’est ainsi qu’a débuté le partenariat entre Withings Health Solutions et Asten Santé.

LA SOLUTION

Les médecins ont la possibilité d’équiper leurs patients d’un Sleep Analyzer Withings, un capteur de sommeil validé cliniquement (3,4). 

 

Le Sleep Analyzer se positionne sous le matelas du patient, une fois installé le patient n’a aucune action particulière à effectuer. Il permet d’enregistrer et d’analyser les nuits des patients tel un laboratoire de sommeil. Il analyse les paramètres vitaux comme la fréquence cardiaque, la fréquence respiratoire ainsi que les mouvements. Cela permet d’obtenir une vue précise des cycles de sommeil, du score de sommeil, de la fréquence cardiaque en continue ainsi que des épisodes de ronflement. Le Sleep Analyzer détecte l’apnée du sommeil et classifie sa sévérité selon la classification de l’Académie Américaine de Médecine du Sommeil, considérée comme une référence mondiale (5). L’un de ses points forts du point de vue clinique réside dans sa capacité à enregistrer des données de manière fiable et non invasive sur de nombreuses nuits, afin de prendre en compte la variabilité inter-nuits de l’index d’apnée-hypopnée, responsable de 20% à 50% d’erreurs de classification de la sévérité du SAOS (6). 

 

Chaque jour, les patients peuvent consulter toutes leurs informations de sommeil directement dans l’application Withings. Cela leur donne la possibilité de suivre les changements dans leurs habitudes de sommeil et ainsi les aider à mieux adhérer à leur traitement.

Les patients peuvent utiliser le Sleep Analyzer pendant 4 mois. Suite à cela ils ont la possibilité de racheter le matelas connecté auprès d’Asten Santé afin de continuer à suivre leur sommeil. 

J’étais sceptique à l’idée d’ajouter un objet supplémentaire dans le quotidien de nos patients. Mais en réalité, c’est tellement facile à utiliser que cela les aide à comprendre l’importance de mettre leur machine pour l’apnée du sommeil.

LES RÉSULTATS

Le partenariat entre Asten Santé et Withings Health Solutions a abouti à un nouveau modèle d’offre serviciel dans le domaine des PSAD. La synergie de l’utilisation de la PPC et du Sleep Analyzer, a permis au patient de devenir acteur de son traitement dès le premier jour, accélérant son acception et son adhésion. D’ailleurs, à la fin du projet, plus de la moitié des patients ont souhaité racheter leur Sleep Analyzer afin de continuer à suivre au plus près leur traitement et son bénéfice.

Notre collaboration avec Asten Santé démontre comment la technologie Withings peut améliorer l'adhésion au traitement de l'apnée du sommeil, tout en offrant aux prestataires de santé à domicile des outils innovants pour se démarquer du marché.

Cette nouvelle offre a permis à Asten Santé de se différencier du marché, auprès des médecins. Comme expliqué par Johan Dupuis, référent national activité respiratoire chez Asten Santé, cela leur a permis de recruter de nouveaux médecins ce qui a conduit au recrutement de nouveaux patients et ainsi répondre au besoin initial d’Asten Santé d’offrir une solution unique aux professionnels de santé.
